Please complete the following forms (attached) for each participating staff member and email them to me Evalyn.thompson@tafeqld.edu.au so the enrolments can be processed before the training date.

1. Student Personal Details form – this must include a USI (Unique Student Identifier number) –

• A USI Factsheet is attached for anyone who doesn’t already have a USI, which explains how to obtain one online

• • • The link to apply for a USI online is: https://www.usi.gov.au/students/get-a-usi

2. Consent to Release Student Information form – this authorises TAFE to email copies of the students’ Statements of Attainment to their employer after successful completion of the course (if applicable).
3. Authority to Invoice form (ATI) listing the fees of $65

 Resources Required

• Enclosed shoes
• Pen and Paper
• ID documents (3 forms of ID need to be supplied otherwise white card can’t be issued on the day)
